Bevacizumab Prescription Guidelines for Ovarian Cancer Treatment

First-Line Treatment Regimen

  • The European Society for Medical Oncology recommends bevacizumab (15 mg/kg) in combination with paclitaxel (175 mg/m²) and carboplatin (AUC 5-6) every 3 weeks for six cycles, followed by bevacizumab maintenance for advanced ovarian cancer (stage III-IV) 1, 2
  • For frail patients, weekly chemotherapy with paclitaxel (60 mg/m²) and carboplatin (AUC 2) can be considered as an alternative 1
  • Bevacizumab improves progression-free survival in stage III-IV ovarian cancer with an ESMO-MCBS score of 3 (4 in high-risk patients) 1, 2

Neoadjuvant Setting

  • Bevacizumab can be considered in the neoadjuvant setting before interval cytoreductive surgery (ICS) with a strength of evidence of II, B 1, 3
  • When interval cytoreductive surgery is not possible and there is no disease progression, three additional cycles of paclitaxel-carboplatin with bevacizumab are recommended 1, 2

Maintenance Therapy Options

  • For BRCA1/2-mutated tumors, maintenance with olaparib plus bevacizumab for 2 years is recommended with an ESMO-MCBS v1.1 score of 3 and an ESCAT score of I-A 1, 4
  • For BRCA1/2-wild-type/HRD-positive tumors, maintenance with olaparib plus bevacizumab for 2 years is recommended with an ESMO-MCBS v1.1 score of 3 and an ESCAT score of I-A 1, 4
  • For HRD-negative tumors, bevacizumab maintenance with a strength of evidence of I, A is recommended 1, 5

Recurrent Disease Setting

  • For platinum-sensitive recurrent disease, bevacizumab (15 mg/kg) can be combined with carboplatin and gemcitabine or carboplatin and paclitaxel, followed by bevacizumab maintenance 6
  • For platinum-resistant recurrent disease, bevacizumab can be combined with paclitaxel, pegylated liposomal doxorubicin, or topotecan for patients who received ≤2 prior chemotherapy regimens 6

Patient Selection Factors

  • Bevacizumab has shown activity in all histological subtypes of ovarian cancer, including less chemotherapy-responsive types like low-grade serous carcinoma and clear cell carcinoma 2, 4

Bevacizumab for Platinum‑Resistant Recurrent Ovarian, Fallopian Tube, or Primary Peritoneal Cancer

Indication & Patient Selection

  • The NCCN Guidelines state that bevacizumab is FDA‑approved for use in platinum‑resistant recurrent disease in combination with paclitaxel, pegylated liposomal doxorubicin, or topotecan for patients who have received two or fewer prior chemotherapy regimens. 7

Dosing & Administration

  • According to the NCCN Guidelines, the recommended bevacizumab dosing regimens are 10 mg/kg intravenously every 2 weeks or 15 mg/kg intravenously every 3 weeks, administered together with a single‑agent chemotherapy; treatment is continued until disease progression or unacceptable toxicity, with no preset limit on maintenance cycles. [8][9]

Chemotherapy Combination Options

  • The NCCN Guidelines advise selecting one of the following single‑agent chemotherapies to combine with bevacizumab:

Maintenance Therapy

  • After completing the chemotherapy component, the NCCN Guidelines recommend continuing bevacizumab as single‑agent maintenance at the same dose and schedule used during combination therapy, persisting until disease progression or unacceptable toxicity. [8][9]

Alternative/Sequential Options

  • The NCCN Guidelines note that PARP inhibitors are not FDA‑approved as monotherapy for platinum‑resistant disease. 8
  • If a patient later achieves a response to a platinum rechallenge, PARP inhibitor maintenance may be considered as an option. 9
