Initial Treatment for Hydatid Cyst
Treatment Algorithm Based on Cyst Characteristics
- The World Health Organization recommends medical therapy with albendazole 400 mg twice daily as the initial treatment for small cysts (<5 cm) 1, 2
- The treatment duration is determined by cyst type and response to treatment, with a recommended cycle of 28 days followed by 14-day albendazole-free intervals for a total of 3 cycles 1, 3
- For larger cysts (>5 cm) or complex cysts, PAIR (Puncture, Aspiration, Injection, Re-aspiration) procedure plus drug therapy is recommended, with medical treatment with albendazole (400 mg twice daily) administered before and after the procedure 1, 2, 4
Treatment Based on Cyst Location
- The liver is affected in 70% of E. granulosus cases, and treatment follows the size-based algorithm above, with continuous albendazole treatment recommended for inoperable cases 1, 2, 4, 3
- For lung cysts, primarily surgical treatment with complete excision, conserving as much lung tissue as feasible, is recommended, with praziquantel given pre- and post-operatively, and albendazole post-operatively 4, 5
Special Considerations
- Late stage cysts (WHO type 4 or 5) may be treated by careful observation with sequential ultrasound scans 1, 2
- E. multilocularis infection requires radical surgical resection due to absence of surrounding membrane, with long, often life-long courses of albendazole required, and higher recurrence rates necessitating close monitoring 1, 2
Pitfalls and Caveats
- The risk of anaphylaxis and cyst dissemination during interventional procedures is significant, and treatment should only be carried out in specialist centers with experience in managing hydatid disease 1, 4, 3
Management of Hydatid Cysts
Treatment Based on Cyst Location
- The American Thoracic Society recommends complete surgical excision with maximum preservation of lung parenchyma as the treatment of choice for lung hydatid cysts, with praziquantel given pre- and post-operatively, and albendazole post-operatively for a prolonged course 6

Special Considerations
- The Infectious Diseases Society of America recommends follow-up imaging (MRI or ultrasound) should be performed at least every 6 months until resolution of cystic lesions 7
