Vancomycin Dosing and Monitoring in Renal Impairment
Initial Dosing and Monitoring Recommendations
- Trough vancomycin concentrations should be obtained at steady state, prior to the fourth or fifth dose, to ensure therapeutic serum concentrations in patients with renal impairment, according to the Infectious Diseases Society of America 1
- For serious infections, such as bacteremia, endocarditis, osteomyelitis, meningitis, pneumonia, and severe skin and soft tissue infections, maintain trough concentrations of 15-20 μg/mL to optimize treatment outcomes, as recommended by the Infectious Diseases Society of America 1
- Monitoring is strongly recommended for patients with renal dysfunction, morbid obesity, or fluctuating volumes of distribution to minimize the risk of vancomycin-induced nephrotoxicity, as emphasized by the Infectious Diseases Society of America 1
Dosing Adjustments in Renal Impairment
- For patients with normal renal function (creatinine clearance >100 mL/min), standard dosing of 15-20 mg/kg every 8-12 hours is recommended, according to the Infectious Diseases Society of America 1, 2
- Maintenance doses should be adjusted based on creatinine clearance, with a daily dose calculated as approximately 15 times the glomerular filtration rate in mL/min, to ensure optimal vancomycin concentrations in patients with renal impairment, as recommended by the Clinical Infectious Diseases guidelines 2
Monitoring and Adjustment of Trough Levels
- Vancomycin-induced nephrotoxicity should be suspected if multiple consecutive high serum creatinine concentrations occur after several days of therapy, and trough levels should be adjusted accordingly to minimize the risk of nephrotoxicity, as recommended by the Clinical Infectious Diseases guidelines 2
- Inadequate monitoring of trough levels in patients with fluctuating renal function can lead to subtherapeutic or toxic vancomycin concentrations, and therefore, regular monitoring is crucial, as emphasized by the Infectious Diseases Society of America 1
