Rheumatology Referral Guidelines

Introduction to Referral Criteria

  • The European League Against Rheumatism recommends referring any patient with joint swelling (synovitis) to a rheumatologist within 6 weeks of symptom onset, and immediately if symptoms persist beyond 4 weeks with inflammatory features 1

Urgent Referral Criteria

  • The American College of Rheumatology suggests referring immediately if a patient has morning stiffness lasting ≥30 minutes, as this may indicate inflammatory arthritis 2
  • The American College of Rheumatology also recommends referring immediately if a patient has elevated inflammatory markers (CRP or ESR) with joint symptoms, as this may indicate an inflammatory condition 2
  • The Mayo Clinic recommends referring immediately if a patient has a positive rheumatoid factor (RF) or anti-citrullinated protein antibodies (ACPA), especially if high-titer, as this may indicate rheumatoid arthritis 3

Standard Referral Timeline

  • The European League Against Rheumatism recommends referring within 6 weeks any patient with persistent joint swelling associated with pain or stiffness, as this may indicate an inflammatory condition 1
  • The European League Against Rheumatism also recommends referring within 6 weeks any patient with undifferentiated arthritis and risk factors for persistent disease, as this may indicate an inflammatory condition 1

Special Circumstances Requiring Immediate Rheumatology Consultation

  • The Society for Immunotherapy of Cancer recommends immediate rheumatology consultation for any patient with grade ≥2 inflammatory arthritis (moderate stiffness/pain limiting instrumental activities of daily living) due to immune checkpoint inhibitor toxicity 4
  • The Society for Immunotherapy of Cancer also recommends immediate rheumatology consultation for any patient requiring >20 mg prednisone daily that cannot be tapered to <10 mg/day within 4 weeks due to immune checkpoint inhibitor toxicity 4
  • The American Society of Clinical Oncology recommends immediate rheumatology consultation for any patient with suspected myositis and muscle weakness, as this is a life-threatening condition 6

Clinical Red Flags That Should Lower Your Threshold for Referral

  • The American College of Rheumatology suggests that improvement with NSAIDs or corticosteroids but not opioids may indicate inflammatory rather than mechanical pain, and thus warrants referral 7
  • The American College of Rheumatology also suggests that stiffness improving with activity is characteristic of inflammatory arthritis, and thus warrants referral 7

Key Principle

  • The European League Against Rheumatism states that rheumatologists are the specialists who should primarily care for patients with inflammatory arthritis, as they achieve earlier diagnosis, earlier treatment initiation, and better long-term outcomes including reduced joint damage and preserved physical function 1
