Pharmacokinetic and Clinical Response Characteristics of Hydralazine

Hemodynamic Response Variability

  • In patients receiving hydralazine for hypertension, blood pressure reduction is unpredictable and the antihypertensive effect persists for a prolonged duration of approximately 2–4 hours, limiting its suitability as a first‑line agent for acute blood pressure emergencies. 1

Onset of Action and Titration Considerations

  • After oral or intravenous administration of hydralazine, the onset of blood‑pressure lowering occurs after a latency of roughly 10–30 minutes; this delayed onset, together with the unpredictable magnitude of response, makes dose titration challenging in acute care settings. 1

Hydralazine Mechanism and Clinical Implications

Hemodynamic Effects

  • This preferential arteriolar dilation increases arteriolar caliber and decreases peripheral vascular resistance with little effect on aortic wave reflections 2

Clinical Implications

  • The direct arterial vasodilation with minimal venous effects explains why hydralazine is therapeutically combined with isosorbide dinitrate in heart failure treatment—hydralazine dilates arteries while isosorbide dinitrate dilates veins, providing complementary hemodynamic benefits 3
  • The American Heart Association suggests hydralazine may help prevent nitrate tolerance when combined with nitrates due to potential antioxidative properties 3
