Colonoscopy Preparation Guidelines

Medication Management

  • Most regular medications, including colchicine, can be continued with clear liquids up until 2 hours before the procedure, with medications taken with minimal clear liquid within 2 hours of the procedure, as recommended by the American Gastroenterological Association 1
  • Colchicine does not require discontinuation before colonoscopy, unlike blood thinners or diabetes medications, according to the Gastroenterology guidelines 1
  • Blood thinners, such as warfarin, should be stopped 5-7 days before colonoscopy if polypectomy is anticipated, with an INR ≤1.5 considered safe for the procedure, as recommended by the American College of Gastroenterology 2
  • GLP-1 receptor agonists should be stopped 1-7 days before colonoscopy, depending on the specific agent, as they delay gastric emptying, according to the American Diabetes Association 1
  • Oral diabetes medications should be held the morning of the procedure, and insulin dose should be reduced according to physician instructions, as recommended by the American Diabetes Association 1

Bowel Preparation Quality

  • Split-dose preparation should begin 4-6 hours before colonoscopy and be completed at least 2 hours before the procedure, with same-day preparation as an acceptable alternative for afternoon colonoscopies, as recommended by the American Gastroenterological Association and the American College of Gastroenterology 1, 2
  • Magnesium-based preparations should be avoided in patients with chronic kidney disease due to potential magnesium toxicity, as suggested by the National Kidney Foundation 2
  • Simethicone can be beneficial as an adjunct to bowel preparation, with doses of 320-1,200 mg shown to improve visualization by reducing bubbles, according to the American Gastroenterological Association 1, 3

Patient Management

  • Patients taking colchicine should be monitored for increased bowel movements during preparation, as colchicine may enhance the effect of bowel preparation agents, and ensure adequate hydration throughout the preparation process 1
  • Patients should follow clear liquid restrictions, taking medications with minimal clear liquid (small sip of water) within 2 hours of the procedure, as recommended by the American Society for Gastrointestinal Endoscopy 1

Special Considerations

  • Colchicine's gastrointestinal effects, such as diarrhea and mild steatorrhea, are unlikely to increase procedural risks, but may interfere with bowel preparation quality, according to the Gastroenterology guidelines 1
  • Iron supplements should be discontinued before colonoscopy, as recommended by the American Gastroenterological Association 2, 1
