Cholesterol Management with Entresto

  • Pravastatin is a preferred option due to its less dependence on CYP3A4 metabolism, reducing potential interactions with Entresto, as recommended by the American Heart Association 2
  • Fluvastatin has different metabolic pathways that make it less likely to interact significantly with Entresto, according to the American College of Cardiology 2
  • Ezetimibe can be considered as an add-on or alternative therapy, particularly in patients who cannot tolerate statins, as suggested by the American College of Cardiology and the American Diabetes Association 3, 4

Medications to Avoid

  • Simvastatin and Lovastatin should be avoided due to their extensive metabolism by CYP3A4 and higher risk of interactions with medications that affect this pathway, as warned by the American Heart Association 2
  • Pitavastatin should be avoided due to potential interactions, as recommended by the American College of Cardiology 2

Monitoring and Special Considerations

  • Baseline liver function tests and CK levels should be checked before initiating combination therapy, and follow-up laboratory testing should be performed 4-12 weeks after initiation and periodically thereafter, as recommended by the American College of Cardiology 5
  • For patients at very high cardiovascular risk who require intensive lipid lowering, consider PCSK9 inhibitors (evolocumab, alirocumab) which work through different mechanisms and may have fewer interactions with Entresto, as suggested by the American College of Cardiology 3
  • In patients with diabetes and heart failure, careful selection of lipid-lowering therapy is particularly important, as emphasized by the American Diabetes Association 4
