Fall Prevention and Osteoporosis Management

Introduction to Fall Prevention

  • The American College of Physicians and other medical organizations recommend exercise interventions combined with multifactorial assessment and targeted interventions to prevent falls and manage osteoporosis in high-risk patients, such as those with dementia, history of falls, and osteoporosis 1, 2

Fall Risk Assessment

  • A patient with a history of falls, dementia, and osteoporosis is at high risk for falls, requiring a comprehensive multifactorial falls risk assessment, including evaluation of gait and mobility, balance, lower limb strength, medication review, cognitive capacity, footwear, and environmental factors 1, 2, 3
  • Initial assessment should focus on key questions about history of falls, fear of falling, and feeling unsteady while walking or standing, with a multifactorial falls risk assessment including the Timed Up and Go test 2

Exercise Interventions

  • Exercise interventions provide moderate net benefit in preventing falls in older adults at increased risk for falls, with a supervised exercise program including gait, balance, and functional training, resistance training, and flexibility training, and consideration of tai chi classes 1, 2, 5, 6, 7
  • The optimal frequency for exercise interventions is approximately 3 sessions per week for at least 10 weeks, with longer durations showing better results, and should be individually tailored and administered by qualified professionals 1, 2, 7

Multifactorial Interventions

  • Multifactorial interventions, including medication review, home hazard assessment and modification, vision assessment, assessment for postural hypotension, and provision of appropriate assistive devices, provide a small net benefit in preventing falls in older adults at increased risk for falls 1, 2, 4, 5

Osteoporosis Management

  • Weight-bearing impact exercise and resistance training should complement pharmacological treatment to promote bone health in patients with osteoporosis 2

Environmental Modifications

  • Home hazard assessment and modification is a critical component of fall prevention in older adults with cognitive impairment, including removal of tripping hazards, installation of grab bars, adequate lighting, and appropriate footwear 2, 4

Education and Caregiver Involvement

  • Involving caregivers in the fall prevention program and providing education about fall risk factors, proper medication management, safe transfer techniques, and recognition of situations that increase fall risk is essential for patients with dementia 1, 4, 8

Implementation Considerations

  • A multidisciplinary approach involving nurses, physical therapists, occupational therapists, and pharmacists is most effective in implementing fall prevention strategies, with regular reassessment of fall risk and intervention effectiveness 4, 8

Monitoring and Follow-up

  • Regular monitoring of fall incidents, medication adherence, exercise program adherence, and bone mineral density, as well as establishing clear communication channels between caregivers and healthcare providers, is essential for effective fall prevention and osteoporosis management 2, 4, 8
