N-Acetylcysteine in COPD Exacerbation Prevention

Patient Selection

  • The American College of Chest Physicians recommends NAC 600 mg twice daily for patients with moderate to severe COPD who have experienced two or more exacerbations in the previous 2 years 1, 2
  • NAC is recommended for patients with ongoing symptoms despite maintenance bronchodilator therapy and inhaled corticosteroids 1, 2

Efficacy Data

  • NAC 600 mg twice daily reduces annual exacerbation rates, with the benefit primarily seen in moderate COPD rather than severe disease 1, 2
  • Time to second and third exacerbations is significantly prolonged with NAC 1, 2
  • The GOLD 2017 guidelines note that mucolytics may reduce exacerbations primarily in patients not receiving inhaled corticosteroids 3

Dosing and Safety

  • The recommended dose is 600 mg orally twice daily for prevention 1, 2
  • NAC is well tolerated with rare adverse gastrointestinal effects 1, 2

Clinical Algorithm for NAC Use

  • Consider NAC 600 mg twice daily for moderate COPD with two or more exacerbations per year despite optimal inhaled therapy 1, 2
  • Patients not on inhaled corticosteroids may derive greater benefit from NAC 3
