Determining Extent of Tinea Corporis

Classification and Diagnosis

  • Tinea corporis is considered extensive when lesions cannot be adequately covered or when multiple widespread lesions are present that would require systemic rather than topical therapy alone, as defined by the American Academy of Pediatrics 1, 2
  • Multiple lesions that cannot be covered with standard dressings indicate extensive disease, according to the American Academy of Pediatrics 1, 2
  • Active lesions confirmed by KOH preparation showing hyphae/arthroconidia or positive culture are indicative of extensive tinea corporis, as stated by the American Academy of Pediatrics 1, 2
  • Solitary or closely clustered lesions that can be covered with gas-permeable dressing are characteristic of localized disease, as defined by the American Academy of Pediatrics 1, 2

Treatment and Management

  • For localized tinea corporis, the American Academy of Pediatrics recommends a minimum of 72 hours of treatment before return to contact activities, and lesions must be covered with gas-permeable dressing 1, 2
  • Disqualification from contact sports is necessary until extensive tinea corporis is adequately treated, as recommended by the American Academy of Pediatrics 1, 2
  • Itraconazole 100 mg daily for 15 days has an 87% mycological cure rate for extensive tinea corporis, according to Praxis Medical Insights 3, 4
  • Terbinafine is superior for Trichophyton tonsurans infections, as stated by Praxis Medical Insights 3, 4

Laboratory Confirmation

  • Laboratory confirmation is essential for extensive disease, and KOH preparation showing hyphae and/or arthroconidia is a diagnostic method, as recommended by the American Academy of Pediatrics and Praxis Medical Insights 1, 2, 4
  • Fungal culture on Sabouraud agar is the gold standard for diagnosis, according to Praxis Medical Insights 4
  • Follow-up sampling until mycological clearance is documented is necessary, as stated by Praxis Medical Insights 4

Extensive Tinea Corporis Definition and Treatment

Clinical Context and Classification

  • There is no universally agreed-upon numerical threshold that defines extensive disease across all dermatology guidelines, with classification being primarily functional, based on whether the infection can be managed with topical therapy and appropriate covering, or requires systemic treatment, as stated by the British Journal of Dermatology 5

Treatment Implications

  • Itraconazole 100 mg daily for 15 days achieves 87% mycological cure rate, superior to griseofulvin's 57%, according to Pediatrics 6
  • Terbinafine is superior for Trichophyton tonsurans infections, the most common pathogen in athletic populations, as reported by Pediatrics 6
