COVID-19 Treatment Guidelines

Introduction to Remdesivir

  • Remdesivir is an antiviral medication used in the treatment of COVID-19, with a recommended loading dose of 200 mg IV on day 1, followed by a maintenance dose of 100 mg IV daily, as recommended by the American College of Physicians, with a strength of evidence considered moderate 1, 2, 3, 4

Remdesivir Administration and Efficacy

  • Remdesivir is administered for a total of 5 days in hospitalized patients without mechanical ventilation and 10 days in patients with mechanical ventilation, with a strength of evidence based on clinical trials and expert opinion 1, 2, 3, 4
  • Remdesivir shows moderate efficacy in hospitalized patients with COVID-19 who require oxygen but do not require mechanical ventilation, with a strength of evidence considered moderate 1
  • Non-hospitalized high-risk patients may receive a 3-day course of remdesivir, with a loading dose of 200mg IV on day 1 and a maintenance dose of 100mg IV daily on days 2-3, as recommended by the American College of Physicians and the FDA 4
  • Pediatric patients ≥40 kg should receive a loading dose of 200 mg IV on day 1, followed by a maintenance dose of 100 mg IV daily from day 2, while pediatric patients <40 kg require weight-based dosing, as recommended by the American Academy of Pediatrics 4

Remdesivir Safety Profile

  • Remdesivir may cause common side effects such as nausea, worsening respiratory function, elevated liver enzymes, and constipation, as well as hypersensitivity reactions, including infusion-related and anaphylactic reactions, in patients with COVID-19 1, 3, 4
  • Absolute contraindications for remdesivir include hypersensitivity to remdesivir components, ALT >10 times upper limit of normal, and eGFR <30 mL/min/1.73 m², as recommended by the American College of Physicians, the National Institutes of Health, and the FDA 4, 1

Remdesivir and COVID Convalescent Plasma (CCP) Combination Therapy

  • For immunocompromised patients with COVID-19, a combination of remdesivir and CCP is recommended, particularly for those with moderate to severe disease who are seronegative or have prolonged viral replication, as per the guidelines from the American Society of Hematology 2
  • High-titer CCP should be administered ideally within 72 hours of symptom onset, with a single dose initially and consideration for additional doses in cases of persistent viremia, providing passive immunity against circulating variants, including those resistant to monoclonal antibodies, with a moderate strength of evidence based on observational studies and case reports 2

Patient Evaluation and Monitoring

  • Before starting treatment, baseline liver function tests, renal function, prothrombin time, SARS-CoV-2 PCR, and serology should be performed, with daily clinical assessment, monitoring for infusion reactions, and serial liver and renal function tests during treatment, to minimize the risk of adverse events and ensure effective treatment, with a high strength of evidence based on clinical trials and expert opinion 2, 3, 4
  • Administration should be done with caution in patients with significant hepatic or renal disease 3

Special Considerations

  • Remdesivir may be considered in pregnant women with moderate to severe COVID-19 who require supplemental oxygen but not mechanical ventilation, where the potential benefit justifies the risk to the mother and fetus, as suggested by the American College of Physicians and other medical societies 3
  • High-risk patients, including those with advanced age (≥60 years), obesity (BMI ≥30), chronic medical conditions, immunocompromised status, and other qualifying comorbidities, may benefit from remdesivir treatment, as recommended by the American College of Physicians 4, 1

Treatment Outcomes

  • Remdesivir treatment may result in a small reduction in mortality for patients requiring supplemental oxygen but not mechanical ventilation, according to the American College of Physicians 4
  • Remdesivir treatment may increase mortality in patients already requiring mechanical ventilation at baseline, as stated by the American College of Physicians 4
  • Remdesivir treatment may result in modest increases in recovery, reductions in hospital length of stay, and reductions in serious adverse events, as suggested by the American College of Physicians 4
  • Remdesivir has demonstrated efficacy in non-hospitalized patients at high risk for COVID-19 progression when administered early in the disease course (within 7 days of symptom onset), with an 87% reduction in hospitalization or death compared to placebo in high-risk outpatients, as supported by the American College of Physicians, the American Thoracic Society, and the PINETREE trial 4, 1
