Diagnostic Approach and Treatment of Distal Fibula Avulsion Fractures

Initial Diagnostic Approach

  • Obtain specialized radiographic views beyond standard ankle series to identify distal fibula avulsion fractures, as they are frequently missed, and the American College of Radiology recommends the use of these views 1
  • The anterior talofibular ligament (ATFL) view is critical and identifies occult distal fibular avulsion fractures in 26% of lateral ankle sprain patients that are not visible on traditional three-view radiographs, according to the American College of Radiology 1

Treatment Algorithm for Acute Avulsion Fractures

  • Immobilize with rigid splinting for 3-6 weeks for most acute avulsion fractures, as recommended by the American Academy of Orthopaedic Surgeons 2
  • Begin active motion exercises immediately following diagnosis to prevent stiffness, which is one of the most functionally disabling complications, and the American Academy of Orthopaedic Surgeons recommends early motion exercises 3, 2
  • The American Academy of Orthopaedic Surgeons also recommends that finger motion does not adversely affect adequately stabilized fractures regarding reduction or healing 3

Follow-Up Radiography

  • No difference exists in outcomes based on frequency of radiographic evaluation for distal fractures treated conservatively, according to the American Academy of Orthopaedic Surgeons 4
  • Consider obtaining radiographs after 2 weeks if the patient experiences new trauma, pain score >6 on VAS scale, loss of range of motion, or neurovascular symptoms, as recommended by the American Academy of Orthopaedic Surgeons 4

Post-Immobilization Management

  • A home exercise program is an effective option after the immobilization period ends, and the American Academy of Orthopaedic Surgeons recommends the use of home exercise programs 5, 2
  • Reevaluate patients with unremitting pain during follow-up, as this may indicate complications, and the American Academy of Orthopaedic Surgeons recommends regular follow-up evaluations 5, 2
