Hypertension Treatment Adaptation

Initial Evaluation and Therapeutic Objectives

  • The target blood pressure should be <130/80 mmHg for most patients, with individualization for elderly patients based on their frailty 1, 2
  • The goal is to reduce blood pressure by at least 20/10 mmHg and achieve the target within 3 months of initiating treatment 1

Therapeutic Strategy by Ethnic Origin

  • For non-black patients, the American College of Cardiology recommends starting with a low-dose ACE inhibitor or ARB, then increasing to a full dose if necessary, and adding a thiazide diuretic or thiazide-like diuretic 1, 2
  • For black patients, the American College of Cardiology recommends starting with a low-dose ARB, then adding a dihydropyridine calcium channel blocker or a thiazide diuretic, and increasing to a full dose 1

Special Considerations for Specific Populations

  • For patients with chronic kidney disease, the guideline society recommends prioritizing an ACE inhibitor or ARB as the essential component of treatment due to their nephroprotective effects 3
  • For elderly or frail patients, the American College of Cardiology recommends considering monotherapy and adapting blood pressure targets based on the degree of frailty 1, 2

Treatment Optimization

  • Simplifying the treatment regimen with a single daily dose and fixed-dose combinations can improve adherence to treatment 1, 2

Resistant Hypertension

  • If blood pressure remains uncontrolled despite optimal triple therapy, the American College of Cardiology recommends considering the addition of spironolactone 1, 3
