Management of Severely Elevated Blood Sugar Levels

Initial Assessment and Treatment Approach

  • For patients with severely elevated blood sugar levels, the American Diabetes Association recommends immediate initiation of insulin therapy along with metformin to rapidly correct high blood sugar and prevent complications 1
  • Patients with blood sugar levels ≥10% should have insulin therapy initiated promptly to rapidly correct high blood sugar, with a recommended initial insulin dosing of 0.1-0.2 units/kg/day or 10 units daily 2, 3
  • Basal insulin should be started while simultaneously initiating metformin, with initial dosing and titration based on blood sugar monitoring 1, 2

Specific Treatment Regimen

  • The American Diabetes Association recommends starting with basal insulin and concurrently initiating metformin, with a low starting dose and gradual titration to minimize side effects 2, 5
  • For patients with symptoms such as frequent urination, excessive thirst, or weight loss, consider adding mealtime insulin coverage 1
  • For patients with blood sugar ≥300 mg/dL, assess for hyperglycemic hyperosmolar nonketotic syndrome 1

Monitoring and Dose Adjustment

  • The American Diabetes Association recommends monitoring blood sugar levels frequently, typically 6-10 times/day initially, and measuring HbA1c every 3 months to assess treatment efficacy 1, 6
  • Insulin doses should be titrated based on fasting blood sugar readings, typically increasing by 10-30% every few days until target is reached 1, 4

Treatment Intensification

  • If glycemic targets are not met with basal insulin plus metformin, advance to combination injectable therapy, with options including adding mealtime insulin or a GLP-1 receptor agonist to basal insulin 4

Long-term Management Considerations

  • Once blood sugar control improves, insulin may potentially be tapered over 2-6 weeks by decreasing doses by 10-30% every few days if the patient is meeting glucose targets 1
  • Regular monitoring of HbA1c is essential to ensure maintenance of blood sugar control, and therapeutic lifestyle modifications including dietary changes and increased physical activity should be emphasized throughout treatment 1, 2, 5
