Out-of-Network MRI Coverage Decision for Student Athlete
Benefit Coverage Analysis
- A full-time student attending school outside the service area qualifies for limited out-of-network coverage, as per the plan's provisions 1

- Clinical findings consistent with potential internal derangement, including joint effusion and pain with medial McMurray's test, are indicative of a meniscal injury, as suggested by the Journal of the American College of Radiology 4
- Pain rated at 5/10, aggravated by movement, and inability to perform some conditioning activities, are symptoms that require medical attention, as noted in the Journal of the American College of Radiology and Praxis Medical Insights 5, 6
Medical Necessity Assessment
- The American College of Radiology recommends MRI knee without IV contrast as the next imaging study after radiographs did not show fracture for evaluation of suspected internal derangement after acute twisting trauma, with a strength of evidence based on clinical guidelines 3, 6
- MRI is the appropriate next imaging study after negative radiographs to evaluate for suspected internal derangement, particularly when meniscal or ligamentous injury is suspected, as per the Journal of the American College of Radiology 4, 3
Coverage Determination
- The American College of Radiology guidelines support the medical necessity of MRI knee for evaluation of suspected internal derangement after acute trauma, with a high level of evidence 3, 6
- Coverage is provided for a covered dependent child who is a full-time student attending school outside the service area, limited to follow-up care received from out-of-network providers, as per the 2025 Large Group Certificate Article V, Section AA, Subsection I, Paragraph 3 1
Coverage Limitations
- Coverage for the out-of-network MRI is limited to 50% of the UCR charge, as determined by GHC-SCW, with a requirement for prior authorization 1
- The follow-up care must be received within 30 days of the initial urgent condition visit, and the plan will cover a maximum of five follow-up care visits per initial urgent condition visit, as per the plan's provisions 1
