Oseltamivir (Tamiflu) Dosing and Administration for Children Weighing > 40 kg

Dosing Recommendations

  • Children weighing more than 40 kg should receive the adult oseltamivir dose of 75 mg per administration【1】【2】【3】.
  • For treatment of influenza, prescribe 75 mg (12.5 mL of the 6 mg/mL suspension) twice daily for 5 days【1】【2】【3】.
  • For post‑exposure prophylaxis, prescribe 75 mg (12.5 mL) once daily for 10 days【1】【2】【3】.

Formulation and Concentration

  • The commercially available oseltamivir oral suspension is formulated at a concentration of 6 mg/mL【1】【2】【3】.
  • Earlier guidelines referenced a 12 mg/mL concentration, but the current FDA‑approved formulation is 6 mg/mL【4】【5】【6】.

Dose Calculation and Administration Technique

  • The required volume for a 75‑mg dose is calculated as 75 mg ÷ 6 mg/mL = 12.5 mL【1】【2】【3】.
  • The 12.5 mL dose should be administered by giving 10 mL followed by an additional 2.5 mL using the supplied oral dosing dispenser【1】【2】【3】.
  • Oseltamivir may be taken with or without food; co‑administration with meals can improve gastrointestinal tolerability【1】【2】.

Timing of Initiation

  • Treatment should be started within 48 hours of symptom onset to achieve maximal efficacy【1】【2】【3】.

Alternative Formulation

  • If the child is able to swallow capsules, a single 75‑mg oseltamivir capsule can be used instead of the oral suspension【1】.

Practical Considerations

  • To ensure an adequate supply for the full 5‑day treatment course, dispense three bottles of the 6 mg/mL oral suspension【1】【2】.
  • No dose adjustment is required for children with normal renal function; however, the dose should be reduced when creatinine clearance is between 10 mL/min and 30 mL/min【1】.

Avoiding Common Errors

  • Do not confuse the suspension concentration; the correct current concentration is 6 mg/mL, not the outdated 12 mg/mL referenced in older guidelines【4】【5】【6】【1】【2】【3】.
