Initial Management of Subdural Hematoma
Immediate Management Algorithm
- All patients with CT-confirmed subdural hematoma require immediate neurosurgical evaluation to assess whether the lesion is life-threatening, regardless of initial clinical presentation 1, 2
- The neurosurgeon will determine surgical candidacy based on hematoma characteristics and clinical status 3
- This evaluation must occur before deciding between conservative management versus surgical evacuation 4, 5
- Subdural hematoma thickness >10 mm is an absolute indication for urgent evacuation 3
- Conservative management criteria include small hematoma (<5 mm thickness) with minimal mass effect and no signs of intracranial hypertension 4, 5
Supportive Management
- Maintain systolic blood pressure >100 mmHg or mean arterial pressure 80-110 mmHg to ensure adequate cerebral perfusion 4, 3
- Maintain euvolemia to optimize cerebral perfusion 4, 3
- Reverse anticoagulation if applicable 3, 5
Critical Considerations
- Never delay neurosurgical consultation in any patient with confirmed subdural hematoma, as even small hematomas can expand rapidly 5
- Avoid hypotension during the evaluation period, as cerebral perfusion pressure must be maintained to prevent secondary brain injury 4, 3
- Do not independently decide on conservative management without neurosurgical input, as this decision requires specialized assessment of multiple factors 4, 5
