Famotidine Administration and Efficacy

Standard Dosing and Administration

  • The American Society of Anesthesiologists recognizes 20 mg IM famotidine as effective for reducing gastric volume and acidity, providing acid suppression for approximately 6 hours, used perioperatively when oral/IV routes are unavailable 1
  • 20 mg IM famotidine is used perioperatively when oral/IV routes are unavailable, providing acid suppression for approximately 6 hours 1

Alternative Routes of Administration

  • In anaphylaxis context, H2-blockers are dosed differently, with ranitidine 50 mg IV in adults being the referenced H2-blocker dose, and H2-blockers are second-line therapy only and never used alone in anaphylaxis 2, 3

Pharmacodynamic Considerations

  • Oral famotidine provides antisecretory activity for 10-12 hours, supporting twice-daily dosing for conditions requiring sustained acid suppression 4

Important Clinical Caveats

  • Tachyphylaxis develops within 6 weeks of continuous H2-receptor antagonist therapy, requiring periodic reassessment of continued need for therapy 1, 5
  • Famotidine does not interfere with clopidogrel's antiplatelet activity, making it preferred over PPIs in patients on dual antiplatelet therapy 1, 4
