Treatment for Gangrene Secondary to Vasculitis

Disease Severity Classification and Treatment Urgency

  • The European League Against Rheumatism recommends immediate high-dose glucocorticoids combined with cyclophosphamide (or rituximab) for the treatment of gangrene secondary to vasculitis, as this represents severe, organ-threatening disease requiring aggressive immunosuppression to prevent further tissue loss and mortality 1, 2
  • Gangrene represents severe vasculitis with critical organ (tissue) failure, placing patients in the highest severity category that demands immediate, intensive immunosuppressive therapy without delay, according to the European League Against Rheumatism 1, 2

Initial Immunosuppressive Regimen

  • The American College of Rheumatology recommends initiating prednisolone 1 mg/kg/day (maximum 60 mg/day) immediately upon diagnosis, with a strength of evidence of high 1, 2, 4
  • The European League Against Rheumatism suggests maintaining high-dose glucocorticoids for one month minimum before initiating gradual taper, based on moderate evidence 4
  • The American College of Rheumatology advises against using alternate-day glucocorticoid therapy, as this significantly increases relapse risk, with a strength of evidence of high 4

Adjunctive Therapies

  • The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines recommend adding plasmapheresis for patients with rapidly progressive disease, dialysis requirement, or diffuse organ failure, with a strength of evidence of moderate 5
  • The European League Against Rheumatism suggests trimethoprim-sulfamethoxazole (800/160 mg on alternate days or 400/80 mg daily) for Pneumocystis jiroveci prophylaxis in all patients receiving cyclophosphamide, based on high evidence 1, 2
  • The American College of Rheumatology recommends bone protection therapy (calcium, vitamin D, bisphosphonates) for all patients on glucocorticoids, with a strength of evidence of high 4

Maintenance Therapy After Remission

  • The Kidney Disease: Improving Global Outcomes (KDIGO) guidelines suggest switching to azathioprine 1-2 mg/kg/day for maintenance therapy, with a strength of evidence of moderate 5, 1
  • The European League Against Rheumatism recommends continuing maintenance therapy for at least 18 months in patients who achieve complete remission, based on moderate evidence 5
