Baclofen Dosing and Monitoring in Elderly Patients

Introduction to Baclofen Use

  • The American Geriatrics Society recommends a starting dose of baclofen in elderly patients of 5 mg three times daily, which is lower than the standard adult starting dose to minimize adverse effects 1

Dosing Considerations

  • Elderly patients rarely tolerate doses greater than 30-40 mg per day, significantly lower than the maximum adult dose of 80 mg daily 1

Adverse Effects

  • Common adverse effects of baclofen include dizziness, somnolence, and gastrointestinal symptoms, according to Praxis Medical Insights 2
  • Abrupt discontinuation of baclofen must be avoided as it can cause central nervous system irritability and potentially severe withdrawal syndrome 1

Alternative Treatment Options

  • For localized muscle spasticity, consider topical treatments or targeted injections rather than systemic baclofen, as suggested by Praxis Medical Insights 2
  • Baclofen may be more appropriate for spasticity than for other types of muscle dysfunction, according to Praxis Medical Insights 2
