Diagnostic Approach and Treatment of Cervical Adenopathy

Imaging and Diagnostic Procedures

  • The American College of Radiology recommends MRI for soft tissue assessment in cervical adenopathy, as it provides detailed evaluation of lymph nodes and surrounding tissues 1
  • The European Society for Medical Oncology suggests that PET/CT may be beneficial for patients with cervical adenopathies from cancer of unknown primary (CUP), as it helps identify the primary tumor site 2

Treatment of Malignant Causes

  • The National Comprehensive Cancer Network recommends external beam radiation therapy (EBRT) to cover the gross disease, parametria, and nodal volumes at risk, along with concurrent platinum-based chemotherapy, for the treatment of metastatic squamous cell carcinoma in the cervical region 3
  • The National Comprehensive Cancer Network also suggests that brachytherapy may be indicated in specific cases of metastatic squamous cell carcinoma in the cervical region, depending on the extent of disease and patient factors 3

Special Considerations

  • The American College of Radiology recommends considering biopsy in cases of lymph node size greater than 2 cm, multiple levels of adenopathy, supraclavicular location, or suspicious imaging findings, as these may indicate underlying malignancy 1
