Evaluation and Management of Hyperhidrosis

Initial Evaluation

  • The American College of Physicians recommends a systematic laboratory evaluation to exclude secondary causes of hyperhidrosis, including thyroid disease and diabetes, which are frequently missed 1, 2
  • Initial laboratory testing should include TSH and free T4 to screen for hyperthyroidism, Hemoglobin A1c to assess for diabetes, complete blood count, complete metabolic panel, serum calcium levels, vitamin D level, and iron studies 1, 2

Focused History and Physical Examination

  • A focused history should assess for secondary causes, including medications such as anticholinergics, dopamine-reuptake inhibitors, diuretics, oral retinoids, and antipsychotics, as well as timing and distribution of sweating 2
  • Associated symptoms such as palpitations, headaches, anxiety attacks, weight changes, heat intolerance, and recent fever or gastrointestinal illness should be evaluated 2, 3
  • Physical examination priorities include looking for hypertension and tachycardia, thyroid enlargement or nodules, signs of heart failure, obesity, and neurological findings 2, 3

Additional Screening and Referral

  • Additional screening should include sleep disorders, such as sleep apnea, and psychiatric assessment for social anxiety disorder 2
  • Consider specialist consultation when abnormal thyroid function, diabetes, suspected pheochromocytoma, signs of Parkinson's disease, stroke, multiple sclerosis, or other autonomic dysfunction, or evidence of heart failure or significant cardiovascular disease are present 2, 3

Critical Pitfalls to Avoid

  • Do not assume primary hyperhidrosis without completing the full workup, as treatable causes such as thyroid dysfunction, diabetes mellitus, medication-induced hyperhidrosis, and pheochromocytoma may be missed 1, 2, 3
