Initiating Acalabrutinib After Completion of Pneumocystis jirovecii Pneumonia (PJP) Therapy
1. Minimum Duration and Completion of PJP Treatment
- Standard high‑dose trimethoprim‑sulfamethoxazole (TMP‑SMX) should be given for 21 days in HIV‑negative patients with moderate‑to‑severe disease before any additional immunosuppressive therapy is considered. [NCCN Guidelines 2017] 3
- Clinical stability (afebrile, stable or improving oxygen requirement, and improving chest imaging) must be maintained for at least 7 days before acalabrutinib can be started. [NCCN Guidelines 2017] 4
2. Hematologic Recovery Requirements
- Absolute neutrophil count (ANC) should be ≥500–1000 µL⁻¹ (ideally >1000 µL⁻¹) prior to acalabrutinib initiation to reduce the risk of compounded infection. [NCCN Guidelines 2020] 1
- Growth‑factor support (e.g., G‑CSF) is recommended for persistent neutropenia until the target ANC is achieved. [NCCN Guidelines 2020] 1
3. Prophylaxis During and After Acalabrutinib
- Continue TMP‑SMX prophylaxis (single‑strength daily or double‑strength three times weekly) indefinitely while on acalabrutinib because of ongoing susceptibility to PJP. [NCCN Guidelines 2020] 1
- Add antiviral prophylaxis (e.g., acyclovir) for herpes‑virus reactivation throughout acalabrutinib therapy. [NCCN Guidelines 2017] 4
- Maintain prophylaxis until CD4⁺ T‑cell count exceeds 200 cells/mm³ for at least 2 months. [NCCN Guidelines 2017] 3
4. Timing for Acalabrutinib Initiation
- All of the following must be satisfied before starting acalabrutinib:
5. Special Considerations for Mantle‑Cell Lymphoma (MCL)
- Delaying acalabrutinib by 2–3 weeks to ensure PJP cure does not materially affect lymphoma outcomes, given the typical median time to response of ~4 weeks with acalabrutinib. [NCCN Guidelines 2020] 6
- Bendamustine‑based regimens cause profound CD4⁺ depletion and markedly increase PJP risk, supporting the need for lifelong PJP prophylaxis in this context. [Hematology‑Oncology Guidelines 2020] 2
- Acalabrutinib demonstrates high efficacy in MCL (overall response rate 91–94 %, median progression‑free survival ≈66 months); however, infection control must precede therapy to avoid fatal complications. [NCCN Guidelines 2017] 3
6. Monitoring After Acalabrutinib Initiation
- Obtain a complete blood count weekly for the first month, then every 2–4 weeks to detect recurrent neutropenia or other cytopenias. [NCCN Guidelines 2020] 1
- Vigilantly assess for new fever, respiratory symptoms, or other infections, adjusting therapy promptly if grade 3–4 neutropenia recurs. [NCCN Guidelines 2020] 1
