Treatment for Vitamin D Deficiency

Initial Treatment Approach

  • For a vitamin D level of 22 ng/mL, treatment should begin with a loading dose of ergocalciferol 50,000 IU once weekly for 8 weeks, followed by maintenance therapy with vitamin D3 1,000-2,000 IU daily to achieve optimal levels above 30 ng/mL, as recommended by the American College of Cardiology 1
  • The initial loading dose regimen effectively raises vitamin D levels in most patients, with a goal to rapidly correct the deficiency 1

Maintenance Therapy

  • Vitamin D3 is preferred over vitamin D2 for maintenance therapy, especially when using longer dosing intervals, with a general rule of thumb that each 1,000 IU of vitamin D daily will raise serum levels by approximately 10 ng/mL 2, 1

Calcium Supplementation

  • Ensure adequate calcium intake of 1,000-1,200 mg daily through diet or supplements, with divided doses of no more than 600 mg at a time for optimal absorption, as recommended by the National Comprehensive Cancer Network 1, 3

Monitoring

  • Recheck vitamin D levels after 3 months of therapy to ensure adequate response, with a target 25(OH)D level of at least 30 ng/mL for optimal health benefits 1, 4

Special Considerations

  • For patients with malabsorption syndromes, higher doses may be required, and for patients with renal disease, vitamin D supplementation should be guided by their specific stage of kidney disease 2, 5, 6
