Osteoarthritis Management Guidelines

Guideline-Based Position

  • The American College of Rheumatology, EULAR, and the American Academy of Orthopaedic Surgeons do not recommend eggshell membrane for osteoarthritis management due to insufficient evidence, as stated in their respective guidelines 1, 2, 3
  • The 2019 ACR/Arthritis Foundation guideline, the 2018 EULAR hand OA recommendations, and the 2022 AAOS knee OA guideline do not mention eggshell membrane as a treatment option, indicating a lack of strong evidence to support its use 1, 2, 3, 4, 5

Clinical Recommendation Algorithm

  • The American College of Rheumatology recommends exercise programs, such as walking, strengthening, and aquatic exercise, as the foundation of treatment for all OA patients, with strong evidence supporting their effectiveness 4, 6
  • The American Academy of Orthopaedic Surgeons suggests weight loss for overweight or obese patients with knee or hip OA to reduce joint load and improve symptoms, based on guideline recommendations 6, 7
  • The American College of Rheumatology strongly recommends topical NSAIDs for knee OA, providing effective pain relief with minimal systemic exposure 6, 7
  • Oral NSAIDs are recommended at the lowest effective dose for the shortest duration, considering cardiovascular and gastrointestinal risks, as suggested by the American College of Rheumatology 6, 7
  • Duloxetine is recommended for patients with inadequate response to first-line treatments or comorbid depression, based on guideline recommendations from the American College of Rheumatology 6

Critical Pitfalls

  • The American College of Rheumatology advises against substituting unproven supplements like eggshell membrane for evidence-based treatments, which may delay or avoid guideline-recommended therapies with demonstrated benefits for morbidity and quality of life 1, 6
  • The American Academy of Orthopaedic Surgeons explicitly does not recommend glucosamine and chondroitin, and eggshell membrane falls into this same category of supplements lacking sufficient evidence for guideline inclusion 6, 7
