Management of Digoxin Therapy in Patients with Hypokalemia

Clinical Manifestations and Management

  • Cardiac manifestations of digoxin toxicity include enhanced atrial, junctional, or ventricular automaticity, often combined with atrioventricular block, in patients with hypokalemia 1
  • The American Heart Association, as published in Circulation, recommends performing an ECG to assess for arrhythmias or conduction abnormalities in patients with suspected digoxin toxicity 2
  • The management of digoxin toxicity involves discontinuing digoxin, monitoring rhythm, and maintaining normal serum potassium levels, with consideration of administering digoxin-specific Fab antibodies for severe toxicity 1
  • Patients on hemodialysis with low predialysis potassium levels have a significantly higher mortality risk when taking digoxin, according to the European Heart Journal 4
  • The American College of Cardiology, as published in Circulation, recommends using lower doses of digoxin (0.125 mg daily or every other day) for patients with risk factors, and avoiding loading doses to minimize the risk of toxicity 3
  • Electrical cardioversion should be planned with caution in patients on digoxin with hypokalemia, with consideration of reducing the digoxin dose for 1-2 days prior to the procedure to avoid inducing ventricular arrhythmias, as recommended by the European Heart Journal and Circulation 2, 4
