Tuberculin Skin Test vs QuantiFERON Gold in Clinical Practice

Key Clinical Scenarios Favoring PPD

  • The Centers for Disease Control and Prevention (CDC) recommends PPD for serial testing programs, such as healthcare workers, because it can demonstrate clear conversion from negative to positive, indicating new infection 1, 2
  • PPD is advantageous in resource-limited settings without laboratory infrastructure, as it requires no laboratory infrastructure and can be administered in any clinical setting with basic supplies 1, 2, 5
  • The CDC guidelines note that PPD allows for two-step testing to detect boosting phenomenon in individuals with remote infection, which is particularly useful in baseline testing of healthcare workers 6, 7

Important Caveats About PPD Limitations

  • The CDC guidelines state that PPD has significantly lower specificity than QuantiFERON Gold, particularly in BCG-vaccinated populations, due to cross-reactivity with BCG vaccine and nontuberculous mycobacteria 1, 5, 8, 9
  • PPD requires a return visit 48-72 hours after placement for reading, leading to incomplete testing when patients don't return, and is subject to reader bias and errors in placement and interpretation 2, 6, 10

When QuantiFERON Gold is Superior

  • The CDC guidelines recommend QuantiFERON Gold for BCG-vaccinated populations, as it is not affected by prior BCG vaccination, making it far more specific in these populations 6, 7, 8
  • QuantiFERON Gold is preferred for one-time screening, such as immigrants or pre-biologic therapy screening, due to higher specificity and no need for return visit 2, 10
