Vitamin D Supplementation Guidelines

Diagnosis and Classification

  • Vitamin D deficiency is defined as serum 25-hydroxyvitamin D levels below 20 ng/mL, while insufficiency is defined as levels between 20-30 ng/mL 1
  • Target serum 25(OH)D level should be at least 30 ng/mL for optimal health benefits, particularly for anti-fracture efficacy 1

Treatment Protocol

  • For adults with vitamin D deficiency, an initial loading dose of 50,000 IU of vitamin D2 or D3 once weekly for 8-12 weeks is recommended 1, 2
  • For severe deficiency, 50,000 IU weekly for 12 weeks followed by monthly maintenance is recommended 1, 2
  • Maintenance therapy with 800-2,000 IU daily or 50,000 IU monthly is recommended after achieving target levels 1, 2

Special Populations

  • For patients with chronic kidney disease, vitamin D supplementation in the form of cholecalciferol or ergocalciferol is recommended to correct 25(OH)D deficiency/insufficiency 3, 4
  • Dark-skinned or veiled individuals may be supplemented with 800 IU/day without baseline testing 1

Safety Considerations

  • Vitamin D toxicity is rare but can occur with prolonged high doses, typically >10,000 IU daily 1, 2
  • The expert panel agreed on an upper safety limit for 25(OH)D of 100 ng/mL 1
  • Patients on anticoagulant medicines should not receive vitamin K supplements alongside vitamin D 3, 4
