Lisinopril Dosing Recommendations for Hypertension and Heart Failure

Initial Dosing and Titration

  • For patients with heart failure, the American Heart Association recommends starting with 5 mg once daily and titrating gradually to a target of 20 mg daily or maximum tolerated dose, as supported by evidence from the Circulation journal 1, 2
  • The European Society of Cardiology suggests allowing 2-4 weeks between dose increases to properly assess response, based on research published in the European Heart Journal 3
  • For heart failure patients, the American College of Cardiology recommends attempting to reach evidence-based target doses for optimal outcomes, particularly when tolerated, as stated in the Journal of the American College of Cardiology 4, 1

Monitoring and Dose Adjustment

  • The American Heart Association advises checking renal function and electrolytes 1-2 weeks after initiation, especially in at-risk patients, such as those with renal impairment, as recommended in the Circulation journal 1
