Corticosteroid Dosing Guidelines

Introduction to CS Dosing

  • The optimum CS dosing schedule is not known and dosing schedules are largely empirical and based on practical experience, as noted by the British Journal of Dermatology 1
  • The European League Against Rheumatism and the American College of Rheumatology strongly recommend against using herbal preparations in conditions requiring steroid tapering, with a focus on evidence-based treatments 2

Tapering Regimens

  • Initial tapering should reduce the dose to 10 mg/day within 4-8 weeks, and once remission is achieved, taper by 1 mg every 4 weeks (or by 1.25 mg using alternate-day schedules) until discontinuation, as suggested by the European League Against Rheumatism and the American College of Rheumatology 2
  • Alternative tapering regimens include:
Weeks Dose
1-2 60 mg daily
3-4 50 mg daily
5-6 40 mg daily
7-8 30 mg daily
9-10 20 mg daily
11-12 15 mg daily
13-14 10 mg daily
15-16 7.5 mg daily
17-18 5 mg daily
  • Another alternative approach is to use the following tapering regimen:
Days Dose
1-7 60mg daily
8-9 50mg daily
10-11 40mg daily
12-13 30mg daily
14-15 20mg daily
16-17 10mg daily
  • A more gradual tapering regimen is:
Weeks Dose
1-2 40mg daily
3-4 30mg daily
5-6 20mg daily
7-8 10mg daily

Special Considerations

  • Methotrexate should be considered as a steroid-sparing agent, particularly in patients at high risk for relapse or prolonged therapy, and can be introduced early, as suggested by the European League Against Rheumatism and the American College of Rheumatology 2, 5
  • Individualized exercise programs should be implemented to maintain muscle mass and function, with guidance from the European League Against Rheumatism and the American College of Rheumatology 2
  • Female patients with difficult-to-control comorbidities, such as hypertension, diabetes, osteoporosis, and glaucoma, may require special consideration when tapering steroids, as suggested by the European League Against Rheumatism and the American College of Rheumatology 2
  • Patients requiring lower cumulative steroid exposure or with challenging adherence to daily oral medication may benefit from alternative formulations or dosing regimens, as suggested by the European League Against Rheumatism and the American College of Rheumatology 2

Monitoring and Prevention

  • Regular follow-up should occur every 4-8 weeks in the first year, and every 8-12 weeks in the second year, as recommended by the European League Against Rheumatism and the American College of Rheumatology 2
  • Monitoring for signs of adrenal insufficiency, such as fatigue, weakness, dizziness, nausea, low blood pressure, and hypoglycemia, is crucial during steroid tapering, as warned by the European League Against Rheumatism and the American College of Rheumatology 2
  • Monitor for steroid-related side effects including cosmetic changes, osteopenia, hypertension, hyperglycemia, and mood changes, as warned by Hepatology and the Annals of the Rheumatic Diseases 6, 2
  • Patients on prednisone >20mg daily for >2 weeks should receive influenza vaccination, pneumococcal vaccination, and vitamin D and calcium supplementation if treatment duration exceeds 3 months, as recommended by the British Journal of Dermatology guidelines and the Centers for Disease Control and Prevention 7, 8
  • Consider calcium and vitamin D supplementation for courses exceeding 3 months to reduce osteoporosis risk, as recommended by various medical guidelines, including the National Osteoporosis Foundation 3, 2, 7
  • For courses exceeding 3 months, the National Osteoporosis Foundation suggests calcium supplementation (800-1000mg daily) and vitamin D supplementation (400-800 IU daily), with a strong level of evidence 8

Disease-Specific Guidelines

  • For grade 2 inflammatory arthritis, the Society for Immunotherapy of Cancer recommends prednisone 20 mg daily for 2-4 weeks, then taper over 4-8 weeks 8
  • For sarcoidosis, the Society for Immunotherapy of Cancer suggests tapering steroids over 2-4 months depending on response 8
  • The European Society for Medical Oncology recommends tapering prednisone over 4-6 weeks for grade 1-2 pneumonitis, and also for grade 3-4 pneumonitis under close monitoring, as well as for hepatitis with close monitoring of liver tests 9
  • For idiopathic inflammatory myopathies, the American College of Rheumatology suggests tapering by 10 mg every 2 weeks until reaching 30 mg/day, then by 5 mg every 2 weeks until 20 mg/day, then by 2.5 mg every 2 weeks after initial high-dose treatment (0.5-1 mg/kg/day) 3
  • For initial treatment of steroid-sensitive nephrotic syndrome, a more gradual taper is recommended after the initial 6 weeks of daily therapy, as suggested by the Kidney International guidelines 10, 11
  • For pericarditis, the European Society of Cardiology recommends prednisone 0.5-1.0 mg/kg daily for 1-2 weeks, then taper for pericarditis 12
  • For immune-related adverse events, the American College of Physicians recommends starting with prednisone 0.5-1 mg/kg/day (typically 30-60 mg daily) for initial treatment, and the National Comprehensive Cancer Network suggests prednisone 1-2 mg/kg/day until symptoms improve to grade 1, then taper over 4-6 weeks 3, 13
