Acute Stroke Management Guidelines
Evidence Against Citicoline Use
- The American Heart Association/American Stroke Association guidelines state that citicoline cannot be recommended for acute ischemic stroke treatment due to lack of consistent efficacy (Grade A recommendation) 1
- The International Citicoline Trial on Acute Stroke found no difference in 90-day global outcome between citicoline and placebo in 2,298 patients with moderate to severe ischemic stroke 1
Recommended Acute Stroke Management
- The American Heart Association recommends immediate brain imaging with non-contrast CT or MRI to determine stroke type and eligibility for evidence-based treatments 2, 3
- The American College of Chest Physicians recommends IV recombinant tissue plasminogen activator (r-tPA) for patients with confirmed ischemic stroke within 3 hours (Grade 1A) or 4.5 hours (Grade 2C) of symptom onset 4, 5
- Early aspirin therapy (160-325 mg) is recommended within 48 hours for patients with acute ischemic stroke who are not receiving thrombolysis 4, 5
- Endovascular thrombectomy should be considered for patients with large vessel occlusions within appropriate time windows 2, 3
Critical Initial Assessment Steps
- Rapid neurological assessment using a standardized stroke scale such as the National Institutes of Health Stroke Scale (NIHSS) is recommended 2, 3
- Initial blood work including electrolytes, random glucose, and coagulation status should be conducted without delaying imaging or treatment decisions 2
- Assessment of vital signs including heart rate, blood pressure, and oxygen saturation is crucial 2
Important Considerations
- Time is critical in acute stroke management, and focus should be on rapid assessment, imaging, and administration of proven therapies rather than unproven neuroprotective agents 2, 3
- The management of physiological parameters, including blood pressure control, is crucial in acute ischemic stroke care 3
- Patients with suspected stroke should be treated in specialized stroke units with coordinated care by interdisciplinary teams with expertise in stroke management 3
Acute Ischemic Stroke Management
Immediate Interventions
- The American Heart Association/American Stroke Association recommends early aspirin therapy (160-325 mg) within 24-48 hours for patients not receiving thrombolysis, after intracranial hemorrhage is excluded, as stated by the International Journal of Stroke 6, 7
- Blood pressure should be lowered to <185/110 mmHg before thrombolysis or thrombectomy and maintained <180/105 mmHg for 24 hours afterward, according to the European Heart Journal and Stroke 8, 9, 10
Blood Pressure Management
- For patients not receiving reperfusion therapy, blood pressure should only be lowered if extremely elevated (>220/120 mmHg), with a modest 10-15% reduction over hours, as recommended by the European Heart Journal 8
