Treatment of Metabolic Alkalosis with Anuria

Immediate Management Approach

  • The American Diabetes Association recommends using isotonic saline (0.9% NaCl) as first-line for volume-responsive metabolic alkalosis, but notes it cannot correct alkalosis without kidney function to excrete bicarbonate 1, 2, 3
  • In patients with anuria, fluid administration risks volume overload without addressing the underlying alkalosis, as stated by the Kidney International guidelines 5

What NOT to Do

  • The Kidney International guidelines advise against using loop diuretics like furosemide, as they can worsen metabolic alkalosis through hydrogen ion loss and are completely ineffective in anuria 5, 6
  • The Gut journal recommends avoiding diuretics, including amiloride, which may help metabolic alkalosis in functioning kidneys but is useless without urine output 5, 6

Critical Monitoring

  • The Diabetes Care guidelines recommend monitoring arterial blood gases and serum bicarbonate every 2-4 hours, as well as serum potassium closely, to manage metabolic alkalosis in anuric patients 3
  • The Praxis Medical Insights guidelines suggest monitoring volume status and signs of overload, as well as cardiac monitoring, to prevent complications in anuric patients with metabolic alkalosis 4
