Desmopressin Therapy Guidelines

Introduction to Desmopressin Dosing

  • The American Academy of Pediatrics recommends desmopressin oral dosing for nocturnal enuresis, using 0.2-0.4 mg oral tablets taken at least 1 hour before sleep, or 120-240 μg oral melt tablets taken 30-60 minutes before bedtime, with mandatory fluid restriction to 200 mL or less in the evening to prevent water intoxication 1
  • The dose is not influenced by body weight or age, and physicians may start with the higher dose and taper down, or use the opposite strategy 1, 2

Nocturnal Enuresis Management

  • Approximately 30% of children become full responders (completely dry) during treatment, and 40% achieve partial response with significant reduction in wet nights 2
  • Desmopressin reduces wet nights by approximately 30-40% during active treatment 2

Critical Safety Requirements

  • Mandatory fluid restriction is essential to prevent water intoxication and hyponatremia, with evening fluid intake limited to 200 mL (6 ounces) or less, and no drinking until morning 1, 2, 3
  • Polydipsia (excessive thirst/drinking) is an absolute contraindication for desmopressin therapy 1, 2, 3

Treatment Monitoring and Duration

  • Regular short drug holidays are mandatory when using desmopressin daily to assess whether medication is still needed 1, 2, 3
  • Before starting desmopressin, perform urine dipstick to rule out glycosuria and proteinuria, and obtain frequency-volume chart for at least 2 days to document nocturnal polyuria 2

Special Population Considerations

  • Desmopressin is not recommended for patients with severe renal impairment (CrCl <30 mL/min), and its clearance is affected by kidney function, requiring close monitoring 1
  • The European Association for the Study of the Liver recommends that desmopressin is not used in isolated liver disease unless concomitant end-stage renal disease is present 1, 4
