Management of Severe Alcohol Withdrawal

Assessment and Treatment

  • A CIWA score of 15 indicates severe alcohol withdrawal requiring aggressive treatment, according to Praxis Medical Insights 1
  • Phenobarbital has a long half-life (80-120 hours) and slow onset of action, which may explain ongoing symptoms despite previous doses, as noted by the American Journal of Kidney Diseases 2
  • The American College of Cardiology is not mentioned in this context, however, Praxis Medical Insights recommends reassessing CIWA score 30-60 minutes after phenobarbital administration to determine response 1

Monitoring and Adjunctive Therapy

  • Monitor vital signs closely, particularly for respiratory depression and hypotension, as advised by the American Journal of Kidney Diseases 2, 3
  • Ensure thiamine 100-300mg/day is being administered to prevent Wernicke encephalopathy, according to Praxis Medical Insights 1
  • Consider adding lorazepam 1-2mg IV/IM every 1-2 hours as needed for breakthrough symptoms, as suggested by Praxis Medical Insights 4

Pharmacology and Safety

  • Phenobarbital works through both GABA enhancement and anti-glutamate activity, however this fact is not supported by a valid citation
  • Phenobarbital has a very long half-life, so effects may accumulate over time - use caution with repeated dosing, as noted by the American Journal of Kidney Diseases 2
  • Check phenobarbital serum levels if available (therapeutic range: 10-25 mg/L; toxic: >50 mg/L), as advised by the American Journal of Kidney Diseases 3

Management of Severe Alcohol Withdrawal

Pharmacological Interventions

  • Benzodiazepines are first-line agents for alcohol withdrawal syndrome, with lorazepam preferred for immediate symptom control in patients with severe AWS, liver failure, or other serious medical comorbidities, as recommended by the American Association for the Study of Liver Diseases 5
  • Thiamine 100-300mg/day should be administered to prevent Wernicke encephalopathy, according to the Clinical and Molecular Hepatology guidelines 5
  • Inpatient treatment is recommended for cases with serious complications such as alcohol withdrawal delirium or seizures, as suggested by the Clinical and Molecular Hepatology guidelines 5
