Fluticasone-Salmeterol Inhalation Treatment Guidelines

Dosage and Treatment Plan

  • The American Thoracic Society recommends determining the starting dosage for asthma patients based on asthma severity, with higher doses for patients with more severe disease or those inadequately controlled on lower doses 1
  • For COPD patients with FEV₁ <50-60% predicted and a history of exacerbations despite optimal bronchodilator therapy, the European Respiratory Society recommends Fluticasone propionate 250 mcg/Salmeterol 50 mcg twice daily 2, 3

Treatment Considerations and Algorithms

  • The American College of Chest Physicians recommends assessing asthma severity based on symptoms, lung function, and exacerbation history, and evaluating risk factors for poor outcomes including FEV₁ <60% predicted, previous exacerbations, and poor symptom control 4
  • For mild persistent asthma, the American Thoracic Society suggests starting with low-dose Fluticasone-Salmeterol (100/50 mcg twice daily) 1
  • The American College of Chest Physicians recommends monitoring COPD patients for pneumonia risk, particularly in those with severe disease, when using ICS-containing regimens 6, 7

Special Considerations

  • The American College of Chest Physicians notes that for COPD patients, the number needed to treat is four patients for one year to prevent one moderate to severe exacerbation with triple therapy versus dual bronchodilator therapy 7
  • The American College of Chest Physicians also reports that the number needed to harm is 33 patients for one year to cause one pneumonia, highlighting a favorable risk-benefit ratio 7
  • High doses of ICS are not typically necessary to achieve optimum benefit in COPD, as shown by a relatively flat dose-response curve, according to the American College of Chest Physicians 7
