Role of Sovateltide in Acute Ischemic Stroke Management

Background and Mechanism

  • Acute ischemic stroke, particularly when caused by large-vessel occlusion, is a severe and life-threatening condition requiring rapid intervention, with current established treatments including intravenous thrombolysis with alteplase and endovascular therapy for large vessel occlusions, as recommended by the American College of Cardiology 1

Current Guidelines and Sovateltide

  • The American Heart Association/American Stroke Association guidelines do not specifically mention sovateltide in acute stroke management, and state that no pharmacological or non-pharmacological treatments with putative neuroprotective actions have demonstrated efficacy in improving outcomes after ischemic stroke 2 3
  • The 2018 AHA/ASA guidelines recommend against neuroprotective agents in general, with a Class III: No Benefit; Level of Evidence A rating 3

Clinical Application and Considerations

  • Sovateltide should not be considered a replacement for established treatments like intravenous thrombolysis or endovascular therapy when those treatments are indicated, as recommended by the American College of Cardiology 1
  • The American Heart Association/American Stroke Association recommends that devices or agents to augment cerebral blood flow should be used only in clinical trial settings, with a Class IIb; Level of Evidence B rating 2
