Beta-Blocker Dosing Post-AMI

Evidence-Based Dosing Thresholds

  • The target dose for bisoprolol is 10mg daily, making the therapeutic threshold approximately 1.25-2.5mg daily, according to the American Heart Association 1
  • Patients receiving >12.5% to 25% of target beta-blocker doses had significantly better survival compared to those receiving lower doses, as recommended by the American College of Cardiology 1
  • The American Heart Association and American College of Cardiology recommend that appropriate beta-blockers for post-AMI patients with left ventricular systolic dysfunction are bisoprolol, carvedilol, and extended-release metoprolol 2, 3
  • Beta-blockers should be prescribed at hospital discharge for all AMI patients without contraindications, as stated by the American College of Cardiology 2
  • The standard bisoprolol dosing is 10mg daily, as established in clinical trials, according to the American Heart Association 1

Clinical Strategy for Dose-Intolerant Patients

  • Identify the specific limiting factor, such as bradycardia, hypotension, bronchospasm, or fatigue, as recommended by the American College of Cardiology 2, 3, 4
  • Rule out contraindications, including decompensated heart failure, advanced heart block without pacemaker, active asthma, or cardiogenic shock risk, as stated by the American Heart Association 2, 3
  • Start with the lowest available dose but target gradual uptitration to at least 1.25-2.5mg bisoprolol daily within 2-3 weeks, as recommended by the American College of Cardiology 4
  • Monitor heart rate, blood pressure, and symptoms at each visit during uptitration, as stated by the American Heart Association 4, 5

Critical Warnings

  • Abrupt beta-blocker withdrawal is associated with 2.7-fold increased risk of 1-year mortality, as warned by the American College of Cardiology 4
  • If the patient is already on 0.625mg, do not stop it abruptly—instead work toward gradual uptitration, as recommended by the American Heart Association 4
