Management of Acute Heart Failure Exacerbation

Initial Assessment and Treatment

  • The European Society of Cardiology recommends immediate initiation of IV loop diuretics at a dose equal to or exceeding the patient's chronic oral daily dose (or 40-80 mg IV furosemide if diuretic-naïve) within 60 minutes of presentation 1
  • Continuous monitoring, including pulse oximetry, blood pressure, respiratory rate, and ECG, should be established immediately upon arrival, according to the European Heart Journal 2, 3
  • Oxygen therapy should be administered if SpO₂ <90% (otherwise use clinical judgment), as recommended by the European Heart Journal 2, 3, 4

Diagnostic Testing

  • A 12-lead ECG should be performed immediately upon arrival to exclude ST-elevation MI and assess for arrhythmias, as recommended by the European Heart Journal and Praxis Medical Insights 1, 2
  • Cardiac troponin should be measured to identify acute coronary syndrome, according to Praxis Medical Insights 1
  • Chest X-ray should be performed to assess pulmonary congestion, as recommended by the European Heart Journal and Praxis Medical Insights 1, 2

Pharmacologic Management

  • The European Heart Journal recommends starting IV loop diuretics within 60 minutes for patients with SBP >110 mmHg, with a dose equal to or exceeding the patient's chronic oral daily dose (or 40-80 mg IV furosemide if diuretic-naïve) 1, 4
  • For patients with SBP <110 mmHg, diuretics remain first-line therapy, but lower initial doses should be used and patients should be monitored closely for hypotension, as recommended by the European Heart Journal 4

Guideline-Directed Medical Therapy (GDMT) Management

  • Praxis Medical Insights recommends continuing existing GDMT in patients with reduced ejection fraction unless contraindications or hemodynamic instability are present 1
  • GDMT should be initiated during hospitalization once clinical stability is achieved, starting with SGLT2 inhibitors and mineralocorticoid receptor antagonists (MRAs), as recommended by Praxis Medical Insights 1

Daily Monitoring Orders

  • Daily monitoring should include strict intake and output measurement, daily weights, vital signs, and clinical signs of perfusion and congestion, as recommended by Praxis Medical Insights 1

Critical Pitfalls to Avoid

  • The European Heart Journal and Praxis Medical Insights emphasize the importance of not delaying diuretic therapy beyond 60 minutes of presentation, as the "time-to-treatment" concept is critical in acute heart failure 1, 2, 3, 4
  • Praxis Medical Insights recommends not discontinuing GDMT for mild renal function decrease or asymptomatic blood pressure reduction unless truly contraindicated 1
  • Beta-blockers should not be started in acutely decompensated patients requiring oxygen or IV therapies, according to Praxis Medical Insights 1
