Diagnostic Approach for Crohn's Disease
Primary Diagnostic Approach
- The European Society of Gastrointestinal Endoscopy (ESGE) recommends ileo-colonoscopy with segmental biopsies and standardized endoscopic scoring as the first-line investigation for suspected Crohn's disease, allowing assessment of disease extent and collection of tissue for histological diagnosis prior to initiating therapy 1, 2
- Ileo-colonoscopy should be performed as the initial diagnostic test when Crohn's disease is clinically suspected and the procedure is safe to perform, including intubation of the terminal ileum with biopsies from both ileal and colonic segments 1, 2
- The ECCO-ESGAR 2019 guidelines and European Society of Gastrointestinal Endoscopy (ESGE) recommendations support this approach 1
Essential Procedural Elements
- Standardized endoscopic scoring systems, such as the Simple Endoscopic Score for Crohn's Disease or Crohn's Disease Endoscopic Index of Severity, should be used to objectively define severity of macroscopic inflammation 1, 2
- Segmental biopsies throughout the colon and terminal ileum are essential to establish histological diagnosis, particularly critical at index presentation before starting medical therapy 1, 2
- Disease extent, distribution pattern, and presence of complications (strictures, fistulae) should be documented 1
Pre-Procedure Considerations
- Patients presenting with chronic diarrhea, abdominal pain, weight loss, and systemic symptoms including fever warrant colonoscopy 3
- Assess for obstructive symptoms, as these may indicate stricturing disease requiring modified approach or alternative imaging first 1, 2
- Evaluate for elevated inflammatory markers and fecal calprotectin levels, which predict positive endoscopic findings 3
Procedure Preparation
- Standard colonoscopy bowel preparation protocols should be used to ensure accurate mucosal assessment and detection of subtle inflammatory changes 1
- A standard colonoscope is appropriate for most cases, but ultrathin colonoscopy may be necessary for patients with known or suspected strictures 1
Diagnostic Algorithm
- The American Gastroenterological Association recommends a step-wise approach for diagnosing Crohn's disease, starting with ileo-colonoscopy, followed by MR enterography if clinical suspicion remains high despite negative or inconclusive ileo-colonoscopy 1, 2, 3
- If both ileo-colonoscopy and MR enterography are negative but suspicion persists, small bowel capsule endoscopy (with patency capsule if obstructive symptoms) may be considered 1, 5, 2, 3
Potential Complications
- Perforation risk is the primary concern during colonoscopy, particularly in patients with severe inflammation, deep ulceration, or stricturing disease 1
- Capsule retention is the main complication of capsule endoscopy, with rates reduced over time through better patient selection and patency capsule use 1
- Balloon-assisted enteroscopy has a perforation risk of 0.15% for diagnostic procedures if needed to retrieve retained capsules or obtain biopsies 1
