TIA Management Guidelines
Risk Assessment and Admission Criteria
- The American Academy of Neurology recommends admitting patients to a stroke unit if they have high-risk features such as acute cerebral infarction on DWI-MRI, large artery atherosclerosis requiring urgent intervention, cardioembolic source identified, crescendo TIAs, known hypercoagulable state, or symptom duration >1 hour at presentation 2, 3, 1
- Patients presenting within 24-48 hours with motor weakness or speech disturbance should be admitted if rapid outpatient follow-up cannot be guaranteed 5, 6
Diagnostic Workup and Observation
- The American Heart Association recommends brain imaging (MRI with DWI preferred; CT if MRI unavailable) within 24 hours, vascular imaging (CTA, MRA, or carotid ultrasound with transcranial Doppler) within 24 hours, cardiac monitoring throughout the observation period, ECG immediately upon arrival, and laboratory tests (CBC, electrolytes, creatinine, fasting glucose, HbA1c, lipid panel, PT/PTT) 3, 2, 1
- Echocardiography (at least transthoracic; can be outpatient if cardiac evaluation otherwise normal) is also recommended 2, 1
Discharge Criteria
- Patients can be safely discharged home after 24 hours if all diagnostic workup is completed and shows no embolic source requiring immediate treatment, no acute infarction on brain imaging, no significant carotid stenosis (≥50%) requiring urgent revascularization, no cardiac source of emboli requiring immediate anticoagulation, no recurrent symptoms during observation, and the patient is neurologically stable with no new deficits 1
Mandatory Discharge Requirements
- The American College of Cardiology recommends initiating antithrombotic therapy (aspirin or dual antiplatelet therapy for high-risk TIA with ABCD2 ≥4 or NIHSS ≤3-5), starting statin therapy for lipid management, addressing blood pressure control, providing stroke education to patient and family, arranging outpatient neurology follow-up within 2 weeks, and giving clear instructions to return immediately if symptoms recur 1, 7
Critical Pitfalls to Avoid
- The American Academy of Neurology advises against discharging patients with crescendo TIAs under any circumstances, relying solely on ABCD2 scores for disposition decisions, delaying carotid imaging in anterior circulation TIAs, and discharging without confirming outpatient follow-up 3, 4, 5, 8, 1, 2
