Alternative Medications for Managing Acute Agitation

First-Line Alternatives

  • For patients with a haloperidol allergy, benzodiazepines or atypical antipsychotics are recommended alternatives for managing acute agitation, according to the American College of Emergency Physicians 1, 2
  • Lorazepam 0.5-1 mg orally four times a day as needed is effective for patients who can swallow, as suggested by the British Medical Journal 3
  • Midazolam 2.5-5 mg subcutaneously every 2-4 hours as needed is effective for patients unable to swallow, as suggested by the British Medical Journal 3
  • Risperidone 0.5-1 mg twice daily orally is a recommended atypical antipsychotic, according to the National Comprehensive Cancer Network 4, 5
  • Olanzapine 2.5-15 mg daily orally or 10 mg intramuscularly is a recommended atypical antipsychotic, according to the National Comprehensive Cancer Network 4, 5
  • Quetiapine 50-100 mg orally or sublingually twice daily is a recommended atypical antipsychotic, according to the National Comprehensive Cancer Network 4, 5

Route of Administration Considerations

  • A combination of oral lorazepam and oral risperidone is effective and comparable to intramuscular treatments for cooperative patients, according to the American College of Emergency Physicians 1
  • Oral atypical antipsychotics provide effective monotherapy with fewer extrapyramidal side effects, according to the American College of Emergency Physicians 1, 2

Clinical Decision Algorithm

  • Assessing a patient's ability to take oral medication is crucial, and if cooperative, consider oral medication, as suggested by the British Medical Journal 3 and the American College of Emergency Physicians 1
  • If uncooperative, consider intramuscular medication, according to the American College of Emergency Physicians 1, 2
  • Considering the underlying psychiatric condition is essential, and atypical antipsychotics are preferred for known psychiatric illnesses, according to the National Comprehensive Cancer Network 4 and the American College of Emergency Physicians 1

Important Precautions

  • For refractory agitation despite high doses of antipsychotics, consider adding lorazepam 0.5-2 mg every 4-6 hours, according to the National Comprehensive Cancer Network 4, 5

Monitoring

  • Reassessing frequently during the first hour after administration to determine if additional medication is needed is crucial, according to the American College of Emergency Physicians 1, 2
