Management of Suicidal Ideation and Aggressive Behavior in Transgender Patients on Testosterone

Immediate Safety Assessment and Crisis Planning

  • The American College of Physicians recommends immediately assessing suicide risk through a structured interview and developing a collaborative crisis response plan, as these interventions reduce suicide attempts by 50% compared to usual care 1, 2
  • A comprehensive suicide risk assessment should include frequency and intensity of suicidal thoughts, specific plans, access to lethal means, and history of previous attempts, as well as current psychiatric symptoms, social support availability, and recent stressors or triggers 2, 3
  • The crisis response plan should be developed collaboratively with the patient and include identification of clear warning signs, self-management coping skills, social support contacts, crisis resources, and scheduled follow-up appointments 1, 3

Psychotherapeutic Interventions

  • The American Psychological Association recommends initiating cognitive behavioral therapy (CBT) focused on suicide prevention immediately, as this reduces suicide attempts by 50% and is the most strongly supported intervention 1, 2, 3
  • Dialectical behavior therapy (DBT) may be considered given the combination of suicidal ideation and aggressive behavior, as DBT specifically addresses emotion regulation, interpersonal effectiveness, and distress tolerance 1, 2, 3, 5
  • Anger management and stress reduction techniques, as well as teaching recognition of emotional triggers and de-escalation strategies, may be implemented to address aggressive behavior 4
  • Problem-solving therapy may be used to improve coping with stressful life experiences 1, 3

Pharmacological Considerations

  • The American College of Physicians recommends avoiding prescribing medications with high lethality in overdose, such as tricyclic antidepressants, given active suicidal ideation 3
  • Benzodiazepines should be used cautiously or avoided, as they may increase disinhibition or impulsivity, potentially worsening aggressive behavior 3
  • Selective serotonin reuptake inhibitors (SSRIs) may be considered over other antidepressants due to their better safety profile 3
  • Ketamine infusion may be considered for rapid short-term reduction in suicidal ideation if symptoms are severe, though evidence for preventing suicide attempts is insufficient 2, 3

Lethal Means Safety

  • The American College of Physicians recommends restricting access to firearms, medications, and other lethal means, as this is a critical intervention regardless of other treatments 6
  • Collaborating with family or support persons to secure or remove firearms from the home, dispensing medications in smaller quantities with frequent follow-up, and addressing access to other means based on the patient's specific plan may be necessary 3

Follow-Up and Monitoring

  • The American Psychological Association recommends scheduling definite, closely spaced follow-up appointments and contacting the patient if appointments are missed 3
  • Sending periodic caring communications, such as postal mail or text messages, for 12 months may reduce suicide deaths and attempts, but sustained contact is necessary 1, 3, 5
