Guideline Recommendations for Clonidine Use and Discontinuation

Tapering and Discontinuation

  • The ACC/AHA Hypertension Guidelines (2017) advise that clonidine must be tapered rather than stopped abruptly to avoid rebound hypertension, tachycardia, and palpitations; abrupt discontinuation is contraindicated. 1 2 3 4 5 6 7
  • The ACC/AHA Hypertension Guidelines (2025) emphasize that sudden cessation of clonidine can trigger rebound hypertension associated with a surge in norepinephrine levels. 8

Resistant Hypertension Management

  • The AHA Scientific Statement on Resistant Hypertension (2018) recommends transdermal clonidine (or guanfacine) as a fifth‑line agent for patients with resistant hypertension who have an elevated resting heart rate (> 80 bpm). 9
  • The same AHA Scientific Statement (2018) advises against oral clonidine tablets in resistant hypertension because the need for frequent dosing increases the risk of non‑adherence‑related rebound hypertension. 9
