Common Pitfalls to Avoid

Rabies Vaccine Dosing and Administration Guidelines (Age‑Independent)

Universal Dose Volume

All individuals, from infants to adults, receive a single 1.0 mL intramuscular dose of either HDCV or PCECV for both pre‑exposure and post‑exposure prophylaxis. The Advisory Committee on Immunization Practices (ACIP) explicitly states that children should receive the same vaccine volume as adults. 4

Injection‑Site Recommendations (Age‑Related Only)

The anatomical site of injection varies by age, while the dose remains unchanged.

Post‑Exposure Prophylaxis (PEP) for Previously Unvaccinated Persons (All Ages)

Standard 4‑dose schedule: 1.0 mL intramuscular vaccine on days 0, 3, 7, and 14. 4

Rabies Immune Globulin (HRIG) Dosing (All Ages)

Weight‑based administration: 20 IU per kilogram of body weight, given concurrently with the first vaccine dose. This dosing is independent of age. 4

Modified PEP Regimen for Immunosuppressed Patients (All Ages)

Extended 5‑dose schedule: 1.0 mL vaccine on days 0, 3, 7, 14, and 28 to ensure adequate seroconversion in immunocompromised individuals. 4

Pre‑Exposure Prophylaxis (PrEP) (All Ages)

Three‑dose series: 1.0 mL intramuscular vaccine administered on days 0, 7, and either day 21 or day 28. The same volume is used for infants, children, and adults. 4

Clinical Caveat – Monitoring Antibody Response in Older Adults

Although the vaccine volume does not change, older adults (> 50 years) may achieve lower antibody titres compared with younger persons; therefore, serologic monitoring is recommended rather than dose reduction. 4
