Sertraline Dosing for Elderly Patients
Initial Dosing and Titration
- The American Academy of Family Physicians recommends starting with 25-50 mg per day in elderly patients, which is lower than the standard adult starting dose 1
- Titrate gradually in increments of 25 mg at intervals of at least one week, as recommended by the American Academy of Pediatrics 3
- Assess response after 4-6 weeks of treatment at a stable dose, as suggested by the American Academy of Family Physicians and other guidelines 1, 3
- The maximum recommended dose is 200 mg per day, though many elderly patients respond to lower doses, according to the American Academy of Family Physicians 1
Monitoring and Adverse Effects
- Monitor closely for clinical worsening, suicidality, and unusual changes in behavior, especially during the initial few months of therapy or at times of dose changes, as recommended by the American Academy of Pediatrics 3
- If discontinuing sertraline, taper slowly over 10-14 days to limit withdrawal symptoms, as suggested by the American College of Clinical Pharmacology 2
Special Considerations
- Avoid rapid dose escalation in elderly patients as this increases the risk of adverse effects, according to the American College of Clinical Pharmacology 2
- For elderly patients with comorbid anxiety, sertraline is effective and can be safely combined with buspirone if needed, as recommended by the American College of Clinical Pharmacology 2
