Treatment of Epigastric Pain Radiating to the Back

Immediate Life-Threatening Causes to Exclude

  • The American College of Radiology recommends CT abdomen and pelvis with IV contrast as the gold standard for diagnosing pancreatitis, perforation, and vascular emergencies, with a sensitivity of 97% for extraluminal gas in cases of perforation 5
  • Acute pancreatitis presents characteristically with epigastric pain radiating to the back and requires serum lipase or amylase ≥3 times the upper limit of normal for diagnosis, with 80-90% sensitivity and specificity 2, 3

Initial Diagnostic Workup

  • The American Heart Association recommends obtaining an ECG to exclude myocardial ischemia, with serial troponins at 0 and 6 hours, to diagnose myocardial infarction which can present atypically with epigastric pain as the primary manifestation, especially in women, diabetics, and elderly patients, with 10-20% mortality if missed 3, 4
  • Checking vital signs for tachycardia ≥110 bpm, fever ≥38°C, or hypotension can predict perforation, anastomotic leak, or sepsis, with a mortality rate of 30% if treatment is delayed 2, 4

Treatment Algorithm by Diagnosis

  • The American College of Gastroenterology recommends non-aggressive intravenous fluid resuscitation (less than 10 ml/kg/hour) for acute pancreatitis, as aggressive fluid protocols increase mortality threefold in non-severe cases 1
  • The American College of Surgeons recommends immediate surgical consultation for laparoscopic or open repair with omental patch for perforated peptic ulcer, which is a surgical emergency with a mortality rate of 30% if treatment is delayed 4

Symptomatic Management

  • The American Gastroenterological Association recommends ondansetron 8 mg sublingual every 4-6 hours for nausea and vomiting, with a baseline ECG due to QTc prolongation risk 2

Critical Pitfalls to Avoid

  • The American College of Cardiology recommends never dismissing cardiac causes based on age alone or "atypical" presentation, and obtaining an ECG within 10 minutes for all patients, as myocardial infarction can present with epigastric pain and has a mortality rate of 10-20% if missed 3, 4
  • The American College of Emergency Physicians recommends avoiding aggressive fluid resuscitation in acute pancreatitis unless the patient has severe disease with persistent organ failure, as this triples mortality in non-severe cases 1
