Ciprofloxacin and Metronidazole Combination Therapy Guidelines

Clinical Indications for Ciprofloxacin and Metronidazole Combination

  • The combination of ciprofloxacin and metronidazole is recommended for complicated intra-abdominal infections, necrotizing fasciitis, and surgical site infections involving the axilla or perineum, as suggested by the Infectious Diseases Society of America 1, 2
  • The American College of Surgeons recommends ciprofloxacin and metronidazole for complicated intra-abdominal infections, particularly for moderate to severe cases, due to its effectiveness against both aerobic gram-negative bacteria and anaerobic bacteria 3, 2
  • The combination is specifically indicated for incisional surgical site infections after surgery of the axilla or perineum, and is effective for surgical site infections of the intestinal or genitourinary tract, according to the Clinical Microbiology and Infection guidelines 1
  • The World Health Organization recommends considering local resistance patterns before using fluoroquinolones, such as ciprofloxacin, for empiric therapy 5

Important Considerations and Cautions

  • The Centers for Disease Control and Prevention suggest that in regions with high rates (>20%) of fluoroquinolone-resistant E. coli, the ciprofloxacin and metronidazole combination may not be appropriate for empiric therapy 6
  • The European Centre for Disease Prevention and Control recommends against routine use of this combination in areas with high prevalence of ESBL-producing Enterobacteriaceae 6, 5

Special Populations

  • The American Academy of Pediatrics recommends that fluoroquinolones, such as ciprofloxacin, generally not be used as first-line therapy in children due to potential adverse effects on developing cartilage, but may be used in children with intra-abdominal infections requiring fluoroquinolone therapy if susceptibility results support this choice 8, 5
