Antibiotic Treatment for Hidradenitis Suppurativa

Introduction to Treatment

  • The American Academy of Dermatology recommends clindamycin 300 mg twice daily combined with rifampicin 300-600 mg daily for 10-12 weeks as first-line therapy for a T2DM patient with hidradenitis suppurativa presenting with an acute boil 1, 2, 3

Treatment Algorithm

  • For moderate disease with abscesses, the combination of clindamycin and rifampicin is specifically recommended, providing both bactericidal action and reducing rifampicin resistance 1, 2, 4
  • Alternative first-line options if combination therapy is unavailable include tetracyclines (doxycycline 100 mg once or twice daily OR lymecycline 408 mg daily) for 12 weeks, although they are generally less effective for acute abscesses compared to clindamycin/rifampicin combination 1, 2, 3

Special Considerations

  • The British Journal of Dermatology suggests screening for cardiovascular risk factors (BP, lipids, HbA1c) as T2DM patients with HS have increased cardiovascular mortality 2, 5

Duration and Monitoring

  • Treat for 10-12 weeks initially, and assess response at 12 weeks using pain scores, quality of life, and lesion count 1, 2, 4, 5
  • Consider treatment break after completion to assess need for ongoing therapy and limit antimicrobial resistance 2, 3

Critical Pitfalls to Avoid

  • Avoid long-term antibiotics without treatment breaks, and balance benefit against resistance risk on an individual basis 1, 4
  • Ensure rifampicin dose is 300-600 mg daily (not lower doses), and clindamycin must be 300 mg twice daily for systemic effect 1, 2

Adjunctive Measures

  • Provide dressings for draining lesions, pain management with NSAIDs, weight management referral, and screen for depression/anxiety 2, 5

When to Escalate Treatment

  • If no response after 12 weeks of clindamycin/rifampicin, consider triple therapy: moxifloxacin + metronidazole + rifampin as second/third-line treatment, or refer to dermatology for consideration of biologics or surgical intervention 1, 2, 5
