Management of Thyroid Nodules with Intermediate Malignancy Risk

Risk Stratification and Management Approach

  • The American College of Surgeons and National Comprehensive Cancer Network recommend that patients with thyroid nodules and intermediate malignancy risk (approximately 20-30% based on molecular testing data) should proceed with either diagnostic thyroid lobectomy or repeat FNA with ultrasound correlation, with the decision based on ultrasound features and clinical risk factors 1
  • The National Comprehensive Cancer Network suggests that high-risk ultrasound features, such as microcalcifications, irregular borders, central hypervascularity, taller-than-wide shape, or extrathyroidal extension, mandate surgical intervention regardless of molecular testing results 2, 3

Clinical Risk Factors and Management

  • The National Comprehensive Cancer Network recommends that high-risk clinical features, such as age <15 years or male gender, history of head and neck irradiation, family history of thyroid cancer or associated syndromes, firm nodule, fixed to adjacent structures, rapid growth, enlarged regional lymph nodes, or vocal cord paralysis, lower the threshold for immediate surgery 2, 3, 1
  • The American Academy of Otolaryngology-Head and Neck Surgery suggests that repeat FNA with on-site cytopathology evaluation can improve diagnostic accuracy and reduce inadequacy rates 4

Surveillance and Follow-up

  • The National Comprehensive Cancer Network recommends measuring serum TSH, as higher levels correlate with increased malignancy risk, and maintaining a low threshold for surgical referral if nodule growth, new suspicious features, or clinical concern develops 3, 6
  • The American Academy of Otolaryngology-Head and Neck Surgery suggests that thyroid ultrasound every 6-12 months for at least 2 years is necessary for surveillance, and that indefinite observation should not continue if nodule demonstrates growth or changing characteristics 4
