PTT Goals for Argatroban and Bivalirudin Monitoring

Argatroban PTT Monitoring

  • The American Society of Anesthesiologists recommends that the target PTT range for argatroban should be 1.5-3 times the patient's baseline value, with a maximum limit of 100 seconds to reduce bleeding risk 1, 3
  • PTT should be measured at least once daily during argatroban therapy to ensure appropriate anticoagulation 1
  • A target value well below 100 seconds must be maintained to reduce the risk of bleeding complications 1
  • Standard aPTT monitoring has limitations with argatroban, particularly in patients with pre-existing prolonged aPTT, post-cardiac surgery, liver failure, or unstable clinical contexts 1

Bivalirudin PTT Monitoring

  • The American College of Cardiology recommends that the target PTT range for bivalirudin is 1.5-2 times the normal level 4, 5
  • aPTT is routinely used to monitor bivalirudin with the same target range as for unfractionated heparin 4
  • For patients with abnormal baseline aPTT values, a diluted thrombin time (TT) can be used as an alternative 4, 6
  • Bivalirudin has a short elimination half-life of about 25 minutes with normal renal function, which may require more frequent monitoring 1, 4

Alternative Monitoring Tests

  • The International Society on Thrombosis and Haemostasis recommends that when aPTT monitoring is unreliable, more specific tests with linear dose-response relationships are recommended, such as Ecarin clotting time (ECT) and diluted thrombin time (TTd) 1
  • Chromogenic anti-IIa testing and diluted TT can be used if an institution has established protocols for these tests 4, 6

Clinical Considerations

  • Dosing adjustments are necessary for patients with liver dysfunction (for argatroban) or renal impairment (for bivalirudin) 1, 4
  • When transitioning from argatroban to vitamin K antagonists (VKA), argatroban should only be discontinued when INR is at least 4 1
